The failure mode in custom software is scope. Someone asks for a quoting tool, and eighteen months later there is a half-finished platform that also does invoicing, stock, CRM and reporting, none of it in use. Across 1,471 IT projects the average cost overrun was 27%, but one in six overran by 200% and ran almost 70% late. Those are not average projects going slightly wrong. They are projects that grew.
